#2 STEP

Download and fill out the required PDF form. Upload the following Documents in the Attach a Document button.

Completed Customer Information Sheet

DTI Business Certificate of Registration (Single proprietor)

SEC Documents (Partnership and Corporation)

Current Mayor’s Permit

BIR Certificate of Registration (2303)

Pictures of Establishment

#3 STEP

Submit the form and wait for the Your Application Has Been Submitted  message to ensure the completion of submitting the form.

Please ensure completion of the application form and sets of documents to be submitted. Incomplete information will delay the approval.
